I'm trying to help out a customer who is not finished building his Proto-Form machine yet but needs some parts made. He describes the mold as a simple block about 20 x 30 x 2 inches tall. Probably MDF and with tapered sides and round corners. Not sure what plastic but he's making concrete molds so probably 1/8 thick
He lives in Alabama and can ship the mold anywhere. PM me for his contact info. if interested

Doug,
Just saw this post. Not sure if you still need help but I have finished the mine about a year ago and can easily mold 1/8 inch in 2x2, 2x3 and 2x4 if needed.

It's a shame that I live in the central-Eastern Alabama (Phenix City, next to Columbus GA) and that I have a vac-form machine, but the heating section is not working as I was expecting, and not good for ABS or the kind, but handles HIPS flawlessly.
